Referrals and using our services
Referrals can be made by a family member, your doctor, an organisation or you can contact us directly. However if you wish to access Australian Government funded services, My Aged Care is your starting point. You will need to be registered with My Aged Care and be assessed to receive funded services.
HART Services are happy to guide you through this process, so give us a call on 02 6628 0265 and we can have a chat to determine whether there is a match between your needs and our service.
Further information about My Aged Care can be found at https://www.myagedcare.gov.au/
Volunteers are an essential part of this program. A variety of rewarding roles are available in both group and individual services.
If you have some spare time and feel you would like to make a difference in someone's life please give us a ring on 02 6628 0265 and enquire.
